
A Melbourne private school has been fined $140,000 after the preventable death of a student on an overseas trip.
Lachlan Cook, a 16-year-old Kilvington Grammar School student, suffered complications from type-1 diabetes while on a school trip to Vietnam in September 2019.
He was taken to the hospital 24 hours after showing symptoms and later suffered a heart attack. He was flown back to Melbourne’s Royal Children’s Hospital, where his life support was switched off in October 2019.
A 2023 coroner’s report found his death was preventable.
WorkSafe charged both Kilvington Grammar and World Challenge Expeditions for failing to protect individuals from health and safety risks.
The Victorian County Court fined Kilvington $140,000 and World Challenge $150,000, with both receiving convictions.
